Qualifications

Prior to the performance of any work Sub-Contractors are required to supply the following
business and insurance information:

W-9 Taxpayer Identification Number and Certification
Current copy of Contractor’s or Business License
Current business phone number and mailing address
Business office Email address

Certificate of Insurance

Limits (See WestScott Certificate)

General Liability

500,000 / 500,000 / 500,000

Workers Compensation

*Insurance certificates must be updated annually
*WestScott Construction, Inc. must be listed as a certificate holder

WestScott Construction, Inc. requires all subcontractors to use the pay application for
payment

Please note that under no circumstances will payment be issued until all qualifications/insurance are met.
